MEDICAID SERVICE COORDINATION (MSC)

Medicaid Service Coordination (MSC) is a central point, from which services are identified, accessed and coordinated. We provide MSC services to individuals with Intellectual/Developmental Disabilities. We assist individuals/families in locating services/resources, monitor service provision, and in understanding theirs rights and entitlements. In addition, MSC also provides advocacy, support and information that facilitate independence in the individual’s community.
MSC uses a person centered planning process in developing, implementing and maintaining an Individualized Service Plan (ISP), which is reviewed every six months. MSC focuses on keeping accurate and current record on service coordination activities and other services provided to the family. MSC is provided in the following languages: English, Spanish, Russian, French, German, Arabic, African dialects, Italian, Punjabi, Bengali, Mandarin, Chinese, Korean etc.
MSC services are provided in the Boroughs of Queens, Brooklyn, Manhattan, Staten Island and the Bronx.
